SGPT is high: what your liver is telling you

SGPT is the number people search for after a report comes back with one red arrow on it. It is an enzyme — alanine aminotransferase, which is why you will also see it written ALT — and it lives inside liver cells. When a liver cell is irritated, a little SGPT leaks into the blood, and the analyser counts it. That is all a raised SGPT is: a leak, measured. It is not a diagnosis, it does not say why, and on its own it does not say how serious.

What does a high SGPT actually mean?

It means liver cells have been irritated recently. It does not say by what. The same value can come from fat sitting in the liver, a viral infection that has already passed, a course of medicine, a heavy weekend, or nothing more interesting than a week of deadlifts. The number tells your doctor to look. The pattern around it tells them where.

Two things matter more than the value itself. The first is how far above the range it sits — a 46 and a 460 are different conversations, and only one of them is patient. The second is direction. The same number twice, ninety days apart, means something quite different from a number that has doubled since March.

How high is high, and what do SGOT and GGT add?

SGPT is almost never read alone. SGOT and GGT are the two markers that turn one number into a shape — which is why all three sit in the same panel rather than being sold separately.

What raises SGPT when the liver is basically fine?

More things than most people expect. Exercise — an unfamiliar workout, a long cricket match, a day of shifting furniture — lifts SGOT more than SGPT, but it lifts both. Medicines, including some ordinary painkillers, antibiotics and supplements bought without a prescription, are a common and reversible reason. A viral fever in the last few weeks can leave enzymes elevated after you feel completely well. Alcoholusually shows itself in GGT first. And weight around the middle is the most common explanation of all in an Indian outpatient department, which is why a raised SGPT so often arrives with a scan line about fatty liver.

None of that is a reason to ignore the value. It is a reason to date it: tell your doctor what the last month looked like, because context is the difference between a repeat test and a workup.

What should I do next?

Three fronts, none of them dramatic. The point is to give the next test something to measure.

Food — ghar ka khana, tuned for the liver. Keep the roti, change the ratio: half the plate sabzi, a fist of dal or paneer, and the fried layer moved from daily to occasional. The two things that shift fastest are sweetened drinks and refined snacks between meals. Nothing is banned; the frequency is what changes.

Movement — no gym required. A 20-minute walk after your largest meal, most days, does more for liver fat than an hour of effort on Sunday. If you have started something new and heavy, tell your doctor before the repeat test, because it can lift the reading on its own.

Habits — the three you can date. Alcohol, counted honestly by week rather than by feeling. Every tablet and supplement written on one list for your doctor to read. And a repeat on the date they give you, so you have two points instead of one — that line is what tells you whether anything you changed is working.

SGPT (ALT) is an enzyme that sits inside liver cells. A raised value means some cells have been irritated recently and leaked enzyme into the blood. It does not name a cause. Fatty liver, a recent viral illness, alcohol, some medicines and hard exercise all raise it. Your doctor reads it beside SGOT and GGT.
Most laboratories call an adult SGPT under about 40 U/L usual, and many use a slightly lower cut-off for women. Ranges differ between analysers, so read the range printed beside your own result rather than one from the internet. Apoorv Pathology prints the range our analyser is calibrated to.
Not for the liver enzymes themselves. The Liver Panel includes a lipid profile, which is usually drawn after 8 to 10 hours without food, so a morning slot is simplest. Water is fine through the fast, and take your regular medicines unless your doctor has told you otherwise.
That is your doctor's call. In practice a mildly raised value is often repeated after about 90 days, because that is long enough for a change in food, weight or alcohol to show up. A sharply raised value is usually looked at much sooner, alongside other tests.
