Iron deficiency: the symptoms most people miss
What does low iron actually feel like?
Mostly like a bad month that will not end. People describe being fine at nine and finished by eleven, needing the handrail on stairs they have climbed for years, and reading the same paragraph three times. Nothing about it announces itself as a blood problem, which is precisely why it gets filed under work, weather, age or a phone habit.
The important framing first, because everything below depends on it: symptoms are context. They tell you and your doctor what is worth measuring. They do not name what you have. Every single sign in the table below is produced by several unrelated things, and a blood report is how the list gets narrowed for a doctor to read.
| What people notice | What it usually gets blamed on | Markers a doctor may read it beside |
|---|---|---|
| Tired by mid-morning, breathless on stairs | Late nights, work, being unfit | CBC, ferritin, iron studies, TSH |
| Hair coming out in the shower drain | Water, shampoo, the season | Ferritin, TSH, vitamin B12, vitamin D |
| Nails brittle, ridged, or spooning inwards | Dishes, detergent, nail polish | Ferritin, CBC |
| Restless, crawling legs at night | A habit, or stress | Ferritin, CBC |
| Craving ice, chalk, mud or raw rice | A quirk nobody mentions aloud | Ferritin, CBC |
| Cold hands and feet in a warm room | Low blood pressure, poor circulation | CBC, ferritin, TSH |
| Mouth ulcers, cracked corners of the lips | Heat, spicy food | Ferritin, CBC, vitamin B12, folate |
Which iron deficiency symptoms get missed most?
Four, in our experience of explaining reports. Restless legs at night gets treated as a personality trait for years. Pica — the pull towards ice, chalk, mud or raw rice — is rarely volunteered because it feels embarrassing, and it is one of the more specific things a person can mention. Hair fall gets chased with shampoos and serums before anyone measures anything. And the cracked corners of the mouth that keep coming back get blamed on heat every summer.
Then there is the one that looks like nothing: exercise that stopped working. Someone walks the same route at the same pace and their heart rate now sits ten beats higher, or the same workload leaves them wrecked. Bodies compensate quietly for a slow drift, so the first honest signal is often effort feeling more expensive than it used to.
Can iron be low while haemoglobin is normal?
Yes — and this is the single most useful thing on this page. Iron leaves the body in a fixed order. Stored iron goes first, so ferritin falls. Then the transport system strains, so TIBC climbs and serum iron drops. Haemoglobin is the last domino, because the body will rob the warehouse for months to keep the shop shelves full.
Which means a CBC that reads inside the range genuinely does not close the question. Someone can have empty stores, a full set of symptoms, and a haemoglobin of 12.4. Doctors ask for ferritin alongside the CBC rather than after it for exactly this reason.
2 · TIBC rises, serum iron drops — more carriers, less cargo. The body is looking harder for iron.
3 · Haemoglobin falls, MCV shrinks — the last stage, and the only one a CBC on its own can see.
Who tends to run low on iron?
Anyone losing blood regularly or growing fast, and anyone whose diet gives iron in the form that is harder to absorb. In practice that is women with heavy or long periods, people who have been vegetarian for years, pregnant and recently pregnant women, teenagers mid-growth-spurt, endurance runners, and anyone with a gut problem that affects absorption. Blood loss from the digestive tract — quiet, slow and painless — matters too, and it is a reason a doctor sometimes looks beyond the blood report.
Two habits are worth naming because they are so ordinary here. Chai or coffee taken with a meal binds a meaningful share of the iron in that meal, and the very same plate eaten an hour apart from the cup behaves differently. And a diet built on refined flour and polished rice carries less iron than the millets and pulses it replaced.
What do the tests cost in Kanpur, and does anything need fasting?
Nothing here needs an empty stomach. The one timing note that matters: serum iron reads higher for a few hours after an iron tablet, so if you are already on one, we slot you before the morning dose — tell us when you book. The Immunity Panel at ₹2,099 bundles 10 tests, including the iron studies, CBC, vitamin B12 and folate that otherwise get booked one at a time; the anaemia testing page lays out what each one adds. Your doctor decides which of them fits your history.
Samples are barcoded at the chair, run on calibrated analyzers and reviewed by a pathologist before release — Dr. Manju, MD Pathology, verifies every report. Results are typically ready in about 6 hours,* and we are registered under the UP Clinical Establishments Act. Home collection is available and charged separately; the fee varies by area and we quote it when you book.
What should you do next?
Food. Three things change how much iron actually crosses from the plate into you, and they cost nothing.
- Put vitamin C in the same meal — nimbu on the dal, amla, tamatar, a slice of orange after lunch.
- Move chai and coffee about an hour away from meals rather than drinking them alongside.
- Keep chana, rajma, palak, ragi and gud in the week's rotation, not just in the occasional meal.
Movement. While stores are low, gentle and daily beats hard and occasional. A twenty-minute walk, stairs at your own pace, two short strength sessions a week if they feel manageable, and a rest day when the day asks for one. Breathlessness that is new, or that turns up at rest, is a reason to call your doctor rather than push through.
Habits. Track the two things people forget to mention: how heavy and how long your periods are, and whether tiredness is worse on particular days. If a doctor has started iron, take it on their schedule — with vitamin C, away from chai and dairy — and retest on their date, because stores refill over months, not weeks. Keep the old report; in a Smart Report the new values sit beside the old ones with what changed written in plain language, and Aira walks you through it on WhatsApp. She explains and coaches; she does not diagnose or prescribe. Every value she talks about has already been verified by Dr. Manju, MD Pathology.
