HbA1c vs fasting sugar: which one is telling the truth

What is the difference between HbA1c and fasting blood sugar?

One is a photograph, the other is a time-lapse. Fasting blood sugar measures the glucose circulating right now, after eight to twelve hours without food — a single, honest frame of one morning. HbA1c measures how much sugar has stuck to your haemoglobin, and because red cells live about three months, it averages every meal, every bad week and every good one across that window.

Which one is telling the truth when they disagree?

Both are. They are answering different questions, and the disagreement is usually the most useful thing on the page.

A fasting sugar sitting comfortably in range next to an HbA1c of 6.2 normally means your mornings are fine and your afternoons are not. Post-meal rises last two or three hours, land nowhere near a 7 am draw, and still get counted in the average. The opposite pattern — a high fasting value with a mild HbA1c — often has a mundane explanation: a late dinner, one short night, an infection that week, or a "fasting" sample drawn at 11 am after eating at 9 pm.

Neither number is a diagnosis. What they are is a pair of coordinates, and a doctor reads them together with your history.

When is fasting sugar the better test?

More often than its ₹60 price suggests. It is the one that knows about today, which matters before a procedure, when symptoms are new, or when a doctor is adjusting something and needs to see the effect this week rather than next quarter. It is also the more reliable of the two when anaemia, a recent transfusion or a haemoglobin variant makes an average built on red cells hard to trust. In pregnancy, doctors generally lean on glucose testing rather than HbA1c altogether.

And it is the one you can repeat often without thinking about cost — useful when the question is "what does my morning look like across a fortnight" rather than "where am I heading".

What should you do next?

Food. Ghar ka khana, tuned to whichever number is out of step. A high fasting value usually points at the last meal of the day — eat dinner earlier, keep it lighter, and put the dal before the roti. A high HbA1c with calm mornings points at the hours after lunch, which is where the 4 pm biscuit and the second cup of sweet chai live.

Movement. A ten-minute walk after the meal that troubles you is the cheapest correction available, and it works on the exact window the fasting test cannot see. Two short home strength sessions a week give the sugar somewhere useful to go.

Habits. Draw both tests the same way every time — same fasting length, same slot, same lab — because half of a confusing comparison is a change in method, not in you. Keep the retest at 90 days, and take both sets of numbers, plus any glucometer readings, to your doctor rather than deciding alone — when it is time to repeat, the HbA1c is ₹550 and needs no fasting.
