Fasting before a blood test: what actually breaks it

Fasting before a blood test means plain water only, for 8 to 12 hours. Water is fine; chai with milk or sugar is not. Never skip a prescribed medicine to fast — tell us the timings instead. At Apoorv Pathology in Kanpur, fasting tests start at ₹60 and home collection slots run 6–11 AM.
How long do I actually need to fast?
The slip says 8 to 12 hours, which sounds like a project. In a Kanpur morning it is simpler than that: finish dinner by 10 PM, drink water whenever you want, take a 7 AM slot. That is nine hours, comfortably inside the window, and you are asleep for most of it.
Longer is not better. Past roughly fourteen hours the body starts running on its own stores, and fasting glucose and triglycerides drift on their own — which makes the number harder to read, not purer. A fast that is too long is as unhelpful as one that is too short.

Ten hours is the easy middle. Dinner at 9:30, a 7:30 AM slot, water through the night. If you overshoot and it is closer to fifteen hours, tell the phlebotomist — it goes on the form so the reading is read in context.
Does chai break the fast — and what about my morning tablet?
This is the question we get most, usually at 6:40 AM at somebody's front door, kettle already on. The rule is calories, not liquid: if it carries energy, the fast is over.
What you had
Breaks the fast?
Why it matters
Plain water, as much as you want
No
Water carries no calories and moves none of the markers on the slip. It also makes the vein easier to find.
Chai or coffee with milk and sugar
Yes
Milk and sugar are food. Glucose and triglycerides start moving within minutes of the first sip.
Black tea or black coffee, no sugar
Best avoided
Almost no calories, but caffeine can nudge glucose and lipids. If you already had a cup, say so — it goes on the form.
Nimbu paani, chhach, coconut water, juice
Yes
Sugar is sugar, including the natural kind. A glass of juice is a small meal to a glucose analyser.
Sauf, elaichi, mouth freshener, one biscuit
Yes
Chewing starts digestion, and "just one biscuit" is still glucose arriving in the blood.
Brushing your teeth
No
Nothing is swallowed, so nothing changes. Brush as usual.
Cigarette, gutka or tobacco
Best avoided
Nicotine shifts glucose and lipids in the hour before a draw. Leave it until after the sample.
Alcohol the night before
Yes, in effect
Triglycerides and liver enzymes can read high for up to 72 hours afterwards. Tell us and we may move the slot.
Prescribed medicines — BP, thyroid, sugar
Ask your doctor
Never skip or delay a dose to fast. Tell us the timings at booking and we slot the draw around them.
Iron or vitamin supplements
Depends on the test
Serum iron reads higher for a few hours after a tablet, so we usually draw before the morning dose.
The medicine row is the one that matters most. A missed blood pressure or thyroid dose is a real risk; a rescheduled blood test is an inconvenience. Tell us at booking what you take and when — we slot the draw around it, and whether anything is paused stays a conversation with your doctor.
Which tests actually need fasting?
Fewer than most people assume. Three of the common ones do:
₹60Blood Sugar Fasting · fast 8–12 h
₹650Lipid Profile · fast 8–12 h
₹760Fasting Insulin · fast 8–12 h
And the ones that do not. HbA1c (₹550) is your average blood sugar over about three months, so a single meal cannot touch it — you can eat breakfast and still give the sample. A CBC (₹300), T3 T4 TSH (₹500), Vitamin D (25-OH) (₹1,400) and Vitamin B12 (₹1,200) are all drawn without fasting. One caution: if a fasting test and a non-fasting test are booked in the same visit, the fast applies to the whole draw. One needle, one set of rules. See the full test list and prices →
What time should I book, and does home collection cost extra?
Home collection runs 6–11 AM across Kanpur, and the timing is not decoration — an early slot is what makes an overnight fast painless. Carry the fast to the door, let the phlebotomist draw, then eat straight away. Plenty of people keep a banana or their tiffin on the table and start on it while the vial is still being labelled.
Home collection is charged separately from the test price and varies by area, so ask on WhatsApp for the charge where you live. Samples are barcoded at your door, run on calibrated analyzers and reviewed by a pathologist before release — Dr. Manju, MD Pathology, verifies every report. Routine tests booked in a morning slot are usually back in about 6 hours.* See how a booking actually works →

What if fasting makes me feel unwell?
If you take insulin or a sugar-lowering tablet, an overnight fast is a conversation with your doctor before it is a booking with us. Most people feel nothing worse than hungry. A little light-headedness that passes when you sit down and eat is common and not an emergency — but some things are.
Call 108 now — do not wait for a test
Chest pain, breathlessness while sitting still, fainting or near-fainting with cold sweat and confusion, sudden one-sided weakness, or heavy bleeding are not reasons to wait for a fasting slot. Call 108 or go to the nearest emergency room now, and eat something on the way if you were fasting.
What do I do next?
Food
Break the fast properly rather than heavily. Ghar ka khana, tuned for a morning that started at six — poha or upma with dahi or a boiled egg, then your chai. Fasting is only there so the reading is clean; it is not a health measure in itself, so there is nothing to extend.
Movement
Skip the hard session on the morning of the draw, and go easy the evening before — heavy training can lift liver and muscle markers for a day or two and cloud an otherwise clean report. A walk is fine. Normal training resumes the same evening.
Habits
Keep alcohol away for three days before a lipid or liver test. When you retest, book the same hour of the morning as last time — comparing a 7 AM draw with an 11 AM one adds noise you do not need. And put the slot in your phone the night before, with a glass of water beside the bed.
This plan is information, not medical diagnosis. Speak with your doctor before acting on any findings.
Once the sample is drawn, the fast has done its job. Common questions
Eight to twelve hours, plain water only. Ten is the comfortable middle — dinner finished by 10 PM, a 7 AM slot, and you sleep through most of it. Longer than fourteen hours is not better: a very long fast moves glucose and triglycerides on its own and makes the reading harder to interpret.
Yes, as much as you like. Plain water changes none of the markers we measure, and being hydrated makes the vein easier to find and the draw quicker. Chai with milk or sugar, coffee, juice, nimbu paani, chhach or a single biscuit all end the fast, because all of them carry calories.
No. Never skip or delay a prescribed dose to make a fast work. Tell us at booking which medicines you take and at what time, and we slot the draw around them. Whether any dose is paused before a test is your doctor's decision, not the lab's.
No. HbA1c (₹550) reflects your average blood sugar over about three months, so one meal cannot move it. CBC (₹300) and T3 T4 TSH (₹500) need no fasting either. Blood Sugar Fasting (₹60), Lipid Profile (₹650) and Fasting Insulin (₹760) do need the 8 to 12 hour fast.
Home collection slots run 6–11 AM across Kanpur, which is exactly what makes an overnight fast easy to keep. Home collection is charged separately from the test price and varies by area — ask on WhatsApp for the charge where you live, and book the earliest slot you can keep.
Aapke sawaal
Doodh aur cheeni wali chai fast tod deti hai — sugar aur triglycerides minton mein badal jaate hain. Saada paani jitna chahein pi sakte hain. Chai sample dene ke turant baad, ghar par.
8 se 12 ghante. Raat 10 baje khana khatam, subah 7 baje ka slot — beech mein sirf paani. 14 ghante se zyada bhookha rehna faayda nahi karta, ulta reading padhna mushkil kar deta hai.
Nahi. Fast ke liye koi bhi prescribed dawai chhodni ya late karni nahi hai. Booking ke waqt bata dein kaun si dawai kab lete hain — hum slot uske hisaab se rakhte hain. Dawai rokne ka faisla sirf aapke doctor ka hai.
